What Is Clinical Judgment in Nursing?
Clinical judgment is the ability to recognize patient cues, analyze clinical data, prioritize patient needs, and make safe nursing decisions — the core skill of modern nursing practice.
Definition: Clinical Judgment in Nursing
Clinical judgment in nursing refers to the thought processes a nurse uses to interpret patient data, determine clinical priorities, select appropriate interventions, and evaluate outcomes. It is a dynamic, ongoing process that draws on nursing knowledge, critical thinking, and real-world experience.
Clinical judgment is not the same as memorizing nursing facts. It requires nurses to integrate knowledge, recognize patterns across complex patient scenarios, and apply reasoning under real clinical pressure — often in rapidly changing situations.
The 6 Steps of Clinical Judgment
Modern nursing education — including NCLEX®-style assessment — is built around a six-step clinical judgment framework:
Recognize Cues
Identify the most important clinical information about a patient — symptoms, trends, lab values, vital signs, and patient reports.
Analyze Cues
Connect the recognized cues to understand what they mean together. Look for patterns, changes from baseline, and relationships between data.
Prioritize Hypotheses
Determine the most likely and most dangerous clinical explanations for the data. Decide what is highest risk and what requires the most immediate attention.
Generate Solutions
Develop evidence-based nursing interventions and care plans that address the prioritized problems safely and effectively.
Take Action
Implement the selected interventions safely, following clinical protocols, scope of practice, and patient-specific considerations.
Evaluate Outcomes
Reassess the patient's response to interventions. Determine if the care plan is working or needs to be adjusted based on new data.
Clinical Judgment and NCLEX®-Style Testing
Modern NCLEX®-style questions are designed to test clinical judgment, not recall. Instead of asking students to recite facts, NCLEX®-style questions present patient scenarios and ask nurses to demonstrate safe, evidence-based decision-making using the six-step clinical judgment framework.
This means nursing students who study only through memorization are often underprepared for NCLEX®-style questions — even when they know the underlying content well.
